Registration and Required Forms

Registration is OPEN for the 2025-2026 School Year! 

New families must secure all items below, including copies of academic records, required medical forms and submit to the school office for review.  An interview with our principal will be scheduled in consideration for admission for grade 1st through 8th.

Please be sure that all required documents are provided to the admissions office:

• Birth Certificate 
• Current/Previous Report Card/Progress Report
• Standardized Test Scores
• Diagnostic or Psychological Evaluation Results, if applicable
• IEP (if applicable) or DCIU test report
• Baptismal Certificate 
• Students are required to have the most up to date vaccines for their age according to the CDC recommended schedule. Student may NOT attend school if they do not have all scheduled vaccinations.
